"Failures have been causing harm and economic loss
ever since the industrial revolution. The creation of the Manchester Steam
Users Association in 1854 was an early attempt to prevent boiler explosions
by using lessons learned from them. Today there are branches of government
and engineering associations, and the insurance industry, whose failure
investigations can help prevent future ones. This presentation will discuss
failure investigations in aviation, construction and consumer products, and
their success and potential in preventing recurrent failures."
